Fitness & Health-Related Information

SENSITIVE INFORMATION DESERVES CARE.

Some application responses—such as information about injuries, allergies, physical limitations, goals, or eating habits—may reveal or allow inferences about your health. We request this information only to understand whether and how general fitness coaching or meal-planning guidance may be structured around your disclosed circumstances.

Wolverine Health Club is a general fitness coaching and meal-planning service. We are not representing that we are a healthcare provider, health plan, healthcare clearinghouse, or HIPAA-covered entity. Our Services do not provide medical diagnosis, treatment, physical therapy, or medical nutrition therapy.

Please do not submit medical records, Social Security numbers, insurance information, payment-card details, genetic information, or other sensitive information that we have not specifically requested. If you have a medical condition, injury, are pregnant, take medication, or require medical nutrition therapy, consult an appropriately qualified healthcare professional.

OUR INTENDED PRACTICE

Information submitted about fitness, nutrition, limitations, or health-related circumstances is intended to be used for reviewing your request, communicating with you, and providing the Services—not for targeted advertising based on sensitive health information.

THE SERVICES ARE FOR ADULTS.

Our personalized coaching application is intended for adults.
